Harm Reduction

  • Assume it isn't methaqualone unless proven otherwise. As above, this is the single most important harm-reduction point for anyone encountering "quaaludes" today outside South Africa's illicit Mandrax supply. You likely don't know what you actually have. Drug-checking services (e.g. WEDINOS: https://www.wedinos.wales) can identify what's actually in a sample.
  • Avoid combining with any other CNS depressant (alcohol, benzodiazepines, opioids, GHB/GBL, other sedatives). This combination was the leading cause of methaqualone-related deaths historically the sedative and respiratory-depressant effects stack, and awareness of "how much you've taken" degrades well before consciousness does.
  • Don't use alone. Given the risk of profound sedation and loss of consciousness, having someone sober present matters.
  • Be aware of the long half-life. Because methaqualone (if genuine) clears slowly and can accumulate, redosing to "boost" a flagging effect carries real overdose risk. What feels like a fading high may not reflect how much is still in the body.
  • If someone becomes unresponsive: put them in the recovery position, monitor breathing, and call emergency services immediately. Consider Naloxone use as it is possible opioids could be sold as methaqualone. Overdose presentation can look different to typical sedative overdoses and can resemble barbiturate toxicity with less cardiac/respiratory depression and more motor disturbance including hypertonia and hyperreflexia. Severe overdose can present as agitation and seizures rather than a classic “floppy, unresponsive” picture.
  • Don't stop heavy, regular use abruptly. If someone has been using heavily and regularly enough to be dependent, sudden cessation can trigger a dangerous withdrawal (including seizures). This needs medical supervision, ideally through a drug service, rather than being managed alone.
  • Route-specific note (South Africa/"white pipe"): Smoking methaqualone with cannabis is reported to produce a faster, more intense effect, which correspondingly narrows the margin for judging dose and increases overdose/accident risk compared with oral use.
